Social worker from fidelity health was here approx 3-4months ago whilst my daughter was not living with me. Only to find that she was assesed without me as a mother knowing what was going on.

I have emails from Jasmin Smith stating she is working with my daughter file and I as a mother have no access to her assessment.

My daughter is going to a epilepsy home but before she goes Annadien from the home requests a social worker report. After speaking to Pusuletso and het interview I was told forms will be ready B Monday to me mailed to the home.

Jasmin Smith emails me stating that medical aid must give permission.

ABSOLUTE JOKE U GET PAID BY MEDICAL TO DO YOUR JOB. WHY WAS I NOT EVEN INFORMED THAT THERE WAS A SOCIAL WORKER SEEING MY DAUGHTER 3MONTHS AGO.

HOW DO U EXPLAIN THAT.

AS PER JASMIN EMAILS I FIND THEM VERY SNOTTY. SHOULD U REQUEST EMAILS I WILL GLADLY MAIL U.

THUS MATTER NEEDS TO BE RESOLVED AS I HAVE LEFT MESSAGES AND JASMIN SMITH DOES NOT CONTACT ME. ONLY EMAILS BAD BUSINESS ETHICS.
