Dismissed but unable to claim my UIF.

I was dismissed on the 2nd of June by Bruce Potter, received my dismissal pack on the 19th of June after my lawyer had requested it. Went to The Labour Department on the 3rd of July to proceed with claiming my UIF and was instructed to go to the companys headoffice for them to provide my UI19 and Blue Card forms. I went there on the 06th of July only to find that the system still says i am employed and they cannot therefore fill my UI19 forms. I was not paid my leave pay on the 25th of June and therefore my debit orders did not go through, now i am unable to claim my UIF because the HeadOffice is unaware of my dismissal. My name is being handed over for collections and i cannot make payment arrangements because i have not received a dismissal letter. The person who chaired the hearing and made the decision to dismiss me has not been in contact whatsoever, i am still waiting for my dismissal letter and for him to let HeadOffice know i was dismissed so i can claim my UIF/Prov.Fund. I have worked for Fedics for 30 years.
