Unclaimed benefits

ive been requesting to be sent a form to complete so that i can claim my benefits. been calling and sending emails and everytime i get told i will be sent the form. we now on a new month and still nothing has been received. everytime i get phone calls confirming my details and telling me i will be sent the form but till date nothing received, is it that hard to get a form sent to me
