Pay just to be re-imbursed

I do not understand why I have an insurance if I have to pay first and then claim back. I pay a large amount for medical insurance to cover for medical emergencies and yet I have to pay and claim back every time. I do not make use of my specialist savings yet there is a problem when I submit an account!
