Expected to pay without invoice

Apparently the medical aid did not cover all costs. I had no problem in paying the outstanding amount, but was rudely told to pay the day of the phone call which was the first time I found out that the medical aid did not cover everything. I would like an official invoice with banking details before I get treated like an unpaying customer! I hope I paid them and not some ********.
