Urgent assistance required

Please assist in getting this matter resolved. I do not know who to direct this compliant.

Our father passed away in June 2021. Liberty requires Section A of the claim form to be completed by the employer as well as the latest payslip. We have been going back and forth to the office requesting for this information but to date nothing has been done. E-mails have also been sent but nothing received.The form was dropped off at the office last year. We have been following up since with both parties but still haven't received anything.

Please assist us in getting Section A of the Liberty form completed and the latest payslip for my late father
