Elexi (Unity Health/ Talksure) not paying?

I tore two ligaments in my ankle and need surgery, I asked Elexi if they will pay for the operation and they said they don't pay for planned procedures (as if I PLANNED to tear a ligament ... or 2!) I sent them the sonar reports and other proof but I have not received a reply from them.
