unauthorized insurance plan

I went to the customer service desk at the Edgars at Fourways mall to enquire about my account balance and to my surprise I was told I had joined Some insurance, hospital plan, dental plan ect ect. I have never Authorised such and haven't even received any communication with regards to these services. I have called the call centre numerous times and have not received an official update but all I get is promises to call me back. This is ********* and extremely frustrating. I need an explanation promptly or I will take this matter up further
