cancelled insurance without permission

I had cellphone insurance which I was paying R40pm. I made a claim last year for a damaged phone and it was replaced. after that I was charged for old fone and new fone taken. I queried this and was reimbursed for incorrect debits. my new fone is now lost and I went to make a claim to find out I dnt have insurance at all. this is not the agreement. disgusted with call centre. I want my fone to be replaced!
