Ui19 form

Hi I'm Anna I was applying for uif couse I'm going on maternity leave so now it appears that I worked at edcon so they need ui19 form from edcon to proceed with the process until I get ui19 form from edcon my application wont get through. I'm just a pregnant woman who needs help. I was asked to call HR edcon but the phone just rings no response
