I was employed by Edcon until 2011/05/31 when I resigned. During the 2nd week of December 2014 I contacted their Head Office requesting completed UI-19 form and Salary Schedule for my maternity benefits claim and the documents were sent to the Department of Labour on the 2014/12/24 only when I queried them. To my surprise the date on both documents was incorrect. I resigned from the company on the 2011/05/31 and on both documents the termination date was 2011/06/01.<br> <br> The Department of Labour queried me about working for two companies at the same time .i.e. on 2011/06/01 I was working for Edcon and my recent employer. I contacted them (Edcon) to rectify the mistake and ever since then my life was hell. I was sent from pillar to post through their Call Centre and electronic mail.<br> <br> To date, almost every second day I get the requested documents though they are incorrect. At some stage my surname is spelled differently and wrongly on both documents and even my termination date was at some point in 2012. The lady who completed the forms all the time recently informed me to contact \my line manager"for confirmation. """
