Insurance Claim

This is not a review but a question. So i lost my phone on Saturday and i bought it with my mother's Jet account but i had an additional card so i used my additional card to buy that phone instead of my mother's card and i insured my phone, so i wanted to know who's details will be needed when i claim for my phone because i used additional card instead of the original card's holder.
