UI19 forms

I worked for Durpro by a printing company and in July we were given saperation papers and I send messages to the wage Clark but he received my messages but don't reply and I told him end of this month I must take my UI19 form to labour department or I will not qualify for unemployment draws I need help and I reported him to other people in the company but it all fell on deaf ears what must I do now his name is Sharniel from Dupro
