urgent cancellation letter

i cancelled my dstv contract and changed it to my spouse name in october 2015 however i was trying to apply for credit and the dstv account still appears under my name. I have called the call center and they have confirmed they cancelled my subscription but they are failing to provide me with a letter that states i have cancelled with them <br> <br> i need the letter as of yesterday <br> <br> frustrated customer
