unauthorized debit order

after my "price lock" deal expired I requested that my banking details be removed as I will now make EFT payments as before. 5 months later Multichoice still do a stop order on my account for my premium subscription. Consultant reason after phoning in again, she not sure why my details was not updated after requested.
