No Service on any level

I requested a DSTV package in January and received a decoder, but no installer came to complete the setup, even after calling 3 times with reference numbers and still no installer after being billed twice. I have cancelled my service and still nobody has been to collect the hardware and return money taken for no service whatsoever.
