i never approve connection for another account

I have not approved the second subscription on my name and I am being bill for it. Last payment made was on the 21/05/2015 for R720. Today I have been cut from watching dstv. I am on the premium package and have not approved any other package on my name. I w want my name to be cleared of whatever charges you'll have against my name. I have been a loyal customer for almost 5 years. Sort this out now.
