Falsely has been registered as an Employee.

I recently check my SARS returns and I found IRP5 from Multichoice while I have never worked for the company and when I check my previous years(2***) IRP5 there's Multichoice listed as my employer and stating different salary amounts each year. As a result SARS I owe money, when I try to contact Multichoice the HR department is never there and when I email them there is no response.
