Extra view costs

In July this year i addrd a 3rd decorder to my package at an additional R95, however it never worked as it didn't pick up the channels. I phoned in numerous times to solve this issue and it was never resolved, i therefore asked for the 3rd decorder to be cancelled. It has not been cancelled and im being billed every month, i phoned it and complained, was promised a refund and also promised that a manager will be in contact with me, but nothing thus far. Im in a price lock of R749, which expires this December i have requested for my DSTV to be cancelled as i refuse to pay for services I've not received. This is unacceptable and i should have received a refund, to date in still being billed R95.
