Dpp: decoder payment plan and Internet issues

What is this dpp decoder payment plan appearing on my credit profile, and affecting my credit score. I am on a month to month contract and have never missed a payment which comes off my account by debit order.

My internet router simcard has been locked and i have been calling non stop and trying to sort it out through the online chat and non of that has helped. apparently I have changed my address which is not true! I am not happy at all!
