Bad Service

I have been contacting Multichoice several times explaining that my uncle passed away and he was paying the account of which my aunt will like to take over but I have been receiving the mail with the form that my passed uncle is supposed to fill in.

How can a dead previous owner fill in the form as i have made it clear and even sent the death certificate to prove that?

Please assist as the communication I am receiving is not satisfactory as all. I have sent all their IDs and death certificate but still no luck at all.
