Very Unprofessional!

Dr. Schalk Burger performed a spinal fusion on me on 28th October 2018. The procedure was partially successful, am still having sciatica issues.

To receive further treat treatment I would require information from him, regarding the type of prosthesis that was inserted and the manufacturer.

This is is only two lines of information!!!!

He sent me a handwritten note which is not readable and I have sent now 8 e-mail over the last 4 month and have left messages on his phone - and there is simply no reply.

I can not understand that a doctor willfully withholds vital information, effectively obstructing me from receiving further treatments.
