Blacklisted because of her fault or Mistake

Good day, i went to see DR R D Mauermann's last year in July for Surgeon. They say I should contact my medical aid first and ask if is gonna pay or not? of which i did and they gave me the number that they wanted in order for them to proceed with the surgeon. The lady at reception gave me a form to fill in and requested my I.d. book to make a copy. After a month i received a letter from Dr Mauermann saying my medical aid did not pay. I phoned the medical aid client services and they say there is no way that they can not pay the claim. i kept on receiving letters from Dr and i kept on calling the medical aid contact services and told me they never received a claim from that Dr M and i phoned Dr Mauermanns reception to tell them that my medical aid didn't receive a claim from them. Then the lady went to check the form that i filled in to compare with the claim form that she submitted and she realized that she made a mistake when she submitted a claim, she filled in the wrong/incorrect membership number and it was too late for my medical aid to accept the claim. I am blacklisted because of that bill by the company called Lamprecht and Spies. '
