Extremely rude and condescending consulation

I was "diverted" by my female GP (of the past 5 years) to this incredibly rude and conde scending "locum" last year October, who had just closed his practice in Standerton. I suffer from debilitating sciatica-like chronic pain caused by nerves being impinged by slipped and/or disintegrating spinal disks L4/5 and T3/4/5, running down my legs, making it feel as though I'm standing in a pot of boiling oil, making life a complete misery. I even got a PSOAS block done two years ago at the same practice , which lasted a mere 10 minutes and the pain returned twice as bad. The "locum" Dr. in question was 15 minutes late("behind schedule" it's called-yeah right) and immediately started reading my thick file. I was trying to save time by informing him of my medical history and what lead to my condition, for which I take chronic schedule 5 pain medication, as well as sleeping and anti-anxiety medication. Whilst I was narrating my life story he hardly looked up, seemed extremely disinterested and then told me to get onto the examination table, where he started performing "reflex tests" which were already repeatedly done in 2013/14 and 15. He then claimed I didn't have sciatica. I also took along a fresh sample of urine (laden with crystals of unknown origin). I figured it out myself-cell casts of haemoglobin from my kidneys. He opened the small sample jar commenting "it reeked of nitrates" and didn't even bother to look at it under a microscope(which I know they have one of at the practice!). The sample of ****** ***** he hardly looked at, merely saying "anyone can see it's blood". He then handed the (not tightly closed) urine sample back, slamming it onto the doctor's desk, to which I offered to mop up with tissues or toilet paper, which I couldn't locate in the myriad of medical sundries behind me on the table between syringes, gloves etc, to which he shouted:"open your damn eyes". Then he did a requested ****** examination with such force that I screamed like a baby. When he started to write out my repeat prescription, I couldn't help but see he was writing out a dosage of 5 years ago, which I pointed out to him. There were 50 pain tablets short. He was furious, telling me:"why are you reading what I'm writing? Who's the professional here?" Then he added that I "was prompting him to tell me things I wanted to hear and that I was obsessively neurotic!" That's where I lost my cool and said that he was being very impolite, if not rude, to which he rep**** :"No-YOU are". I was told to come back 2 months later after this "consultation" for a PSA level blood test to rule out prostate cancer. Needless to say the follow up never happened. On my way home I had that urine sample tested at Ds-Chems clinic for "nitrates"-NONE was found only BLOOD! I wrote an email that very night to my female GP, which turned out to be the main doctor's email address. He rep**** that Dr. Herbst was a year long friend of his, very professional and that nobody's ever complained about his service. I beg to differ. When I went to pay the bill a few days later(against my will) I asked my files back to go elsewhere, to which I was told that Dr's files remain the property of the doctors and was given back papers and records I(!) had supp**** them from Steve Biko Hospital as well as privately self paid for blood tests. I also noted when signing for these, that there was a block where it clearly said, if you would like to make use of our services in the future you are welcome. When trying to make an appointment with my female GP, today for my 6-monthly chronic medication repeat, as well as showing the latest ****** ***** samples(6 months later) I waited the whole day for her to return my call, only for me to have to call back at 16h45 and speak to the other receptionist, who immediately recognized my voice, asked me how I was doing, and when I asked if my female GP would still see me after the previous ********** "consultation" I was told that the male Dr in charge now decides, which doctor a patient gets to see. I find this very strange and hardly amusing. This practice has made thousands of Rands from me, only for me to be treated like the village idiot. Is there anywhere one can report such an exceptionally rude Dr?
