INCORRECT DETAILS SUBMITTED IN OFFICIAL REPORT

I was referred to this 'specialist' my a new GP. I spent 2 hours detailing my history while he completed questionnaires on the laptop. When the report was received he has confused and mixed up all my medical history. I asked him to amend the report, but he insist i must go back and pay again to have him correct the report. The knock on effect is that this report is taken as gospel by an insurance company who pretty much think I am dead! All I expect is for the Dr to get the facts correct and re submit the report.
