Unproffessionalism and poor services

I went to consult with Dr DJ Monnakgotla (Practise NO: ********** ). Tel ********** or Mobile ********** . He is based in Kaalfontein Ext 17 in Ebony Park (Midrand). After waiting for an hour unattended in his consulting room. He asked one of her nurses to draw my blood of which it was a terrible experience, I dont even know if the nurse knew how to do it because my whole arm died and i was feeling very dizzy. Then the doctor ordered the nurse to stop and that i should come the following day to do it. Well, the second day was a success. I was told they will contact me to come and fetch my results and they never did. I had to phone to ask they my results are back or not. The second issue is my sick note. After numerous attempts and countless phone calls, I am still chasing for the note even today. They keep promising that i will get it on my email but they are not sending and i have to submitt to our HR department at work. He (the doctor)is now not taking my calls and not responding to my text.

Please can you assist.
