DR DJ Crafford Practice **********ly adding costs to my account

I have asked this practice on numerous occasions to close my account. They keep sending me statements showing an outstanding amount, which I have proven to them over and over again (by proof of payment and bank statements) that my account was settled. They simply do not engage in conversation or discussion and simply keep sending statements. There has not been any effort from their side to show that their is in fact monies owed.

Furthermore, they allow my x-wife access to my account, simply charging costs for which I am then expected to settled, this after I have explicitly informed them over and over again that they can not add 3rd parties' costs to accrue to me, this is *****.
