its been 3 months that I'm waiting for the Dr to send through the UI-19 form to the department of labour.<br> when he does fill it out and send it through its incomplete or filled out incorrectly.<br> this has given me a lot of troubles as my accounts are in arrears as I haven't received my uif maternity benefit.<br> I now have a bad itc record.<br>
