Refusal to Sign UI19 form for Ex employee

I had worked for Dr B Goldberg for a month when my fianc?s grandfather had passed away so we had to go away for the funeral. I was away from work for two days, the afternoon of the second day I received a call from one of the Dr's other employees telling me that the Dr said I was irresponsible and I shouldn't come back.<br> so I didn't go back, almost two years later I'm on maternity leave and need the Dr to sign the UI-19 form to claim my maternity uif benefits and I get no cooperation from the dr.<br> I faxed through the form and never got it back.<br> I have been to the consulting rooms twice and they didn't see me, he asked the receptionist to tell me that he has to first check with his bookkeeper before he signs any forms because i hadn't worked the full month.<br> I called his bookkeeper who then sent through the forms to him and asked him to sign it and hand it to me, however he only handed the sars IRP5 form to me. he didn't want to hand in the UI-19 form coz it has his wife's ID Number.<br> I haven't been paid my maternity benefit yet and I'm even back at work, however this has impacted negatively on my ITC record as I had no income for four months.<br> this is delaying the process.
