Poor service

My mother have policy with Dove Funeral since 2017 she's paying every month and she also checked if everything is fine with her account she was told that her account is up to date and the the names and ID numbers of her dependants where correct ,but when she claims now cause my cousin passed away she was told that my cousin was removed from her list no communication was made regarding this only now she's claiming.
