Dear Sir/Madam,

I refer to the recent passing of Willem Johannes Jacobus Abraham Coetzee I. D ‪***‬, for whom your establishment (DOVES INNESDAL) rendered funeral services(14 May 2025). In order to finalise and process the related death insurance claim, the BI 1663 (Notice of Death/Stillbirth) form is required.

You are hereby formally requested to provide a copy of the BI 1663 form by close of business 3 November 2025 to enable the insurer to proceed with settlement of the claim.

Please note that the release of this document does not constitute a breach of the Protection of Personal Information Act (POPIA). Section 11(1)(c) of POPIA allows for the processing and disclosure of personal information where such processing is necessary to comply with an obligation imposed by law, or under Section 11(1)(f), where it is necessary for the legitimate interests of the data subject or a third party in this case, the lawful processing of a death insurance claim.

The BI 1663 form is an official government document specifically created for administrative and legal purposes following a death, and its provision to an authorised party for insurance settlement purposes is lawful and appropriate.

Should you fail to provide the requested document within the requested time, and this causes delay or non-payment of the insurance claim, please note that I will have no alternative but to institute a formal claim for damages against Doves Funeral Home for any financial loss suffered as a result of your non-compliance.
