DMC 5 be removed from my credit profile

Hi my name Thapelo magodielo ,DMC 5 is reflecting on my credit report ,what is this all about ? All my accounts reflect on my credit profile ,I just don't understand what DMC 5 on my credit profile is all about , because i have never made an account with this people .Please assist on this because it's affecting my credit profile
