STATEMENT ACCOUNT INCLUDING MY PHYSICAL ADDRESS

I am fighting for my life with DMC to send me my Account statement that includes my physical address. I have spoken to Cindy Naidoo twice,I have sent an email and have called to no satisfaction. WHAT IS THE MATTER FORREAL WHY IS IT SO HARD TO GET A STATEMENT INCLUDING MY PHYSICAL ADDRESS??? I am taking this up with the OMBUDSMAN this is madness!
