Vitality Cancellation Query

I am unable to make successful contact with Discovery Vitality to obtain the Vitality Cancellation Form. I have tried every avenue on the website and the Help, Call Centre and Search functions all loop back to an automated list of FAQ and queries to read through. In the Vitality Rules they make reference to a Cancellation Form which is seemingly unavailable or certainly I cannot find it nor access it.
