Unable to provide clear information

I recently submitted a health assessment, including evidence of the test, and was now told that points cannot be awarded because the date I entered for the test is incorrect. The date I entered corresponds to the date that is stated on the lab report that reflects the test undertaken. Please specify what date you want to appear on the test. Although, this kind of behaviour is ridiculous nitpicking. All dates are already clearly stated on the report. Please learn how to write clear instructions if dates are important to you.
