Unable to claim vitality points

I have tried multiple times to claim the vitality points for my child's vaccinations this year but am told information is missing. I have attached photos of the vaccination record. As such all information that is requested has been supp****. It is unclear what information Discovery requires to finalise these claims. This system does not work particularly well.
