SWAPPING FROM LIFE VITALITY TO HEALTH VITALITY

On the 01st April 2015 I have joined discovery health of which on the time when I joined the health discovery I therefore decided to move from life policy vitality to health vitality. I have then await for confirmation that my health vitality has been activated refer MailRef#*** attached email received on 21 May 2015. On the 08 June 2015 I have sent a letter to cancel my life policy vitality of which on the 10 June 2015 received a confirmation refer email attached MailRef#***. On the 27 June 2015 received a Vitality Newsletter with no reference but refer attached email (Ups for Grabs Uber). On the 30 July 2015 I have received Vitality Newsletter of which I assume that my health vitality is active refer attached MailRef#***. Then on the 02 October 2015 received a survey about why I have left vitality of which I did not since I knew that I have swapped from Life Policy vitality to Health vitality. On the 26 October 2015 I was called by Virgin Active consultant that my vitality has been cancelled.
