No points awarded as they should have!

Discovery has carried out a health check on the 07/09/2017 at my place of work. They charge for this and theres no points loaded on vitality as yet. I manually submitted my HIV test and they say they cannot award points as they call me by my preferred name Shahid and the nurse put Mahomed Hoosen. Its not my fault that the Nurse did not put the full on the form. it is attached. mail ref ********** 520. Am I really gonna ***** HIV results?
