Misleading on the Vitality 3 months free trail and no cash back

I took out Vitality on the 1st of March 2025 on 3 months free trail basis, The first payment for it was due to be on the 1st of June 2025 and they took it on the 1st of May 2025 meaning i the one more month free trail. additionally when i took it out i was informed i will get 25% back if i purchase from Clicks and checkrs and till this day i have not received it, when i called i was informed it was due to do a health test first in order to qualify which is something i was never told and it is also not on their Discovery bank and health App as it states i will get 25% cashback and after the test it will increase to 45% cash back. i raised a complaint and have not been assisted with this complaint and no manager has even bothered to call me back, I want my cash back to be added on my discovery bank and request for a refund for the direct debit you took without depleting my 3 month trail.
