Discovery Vitality points and chronic conditions

I need to understand the strategy of Discovery Vitality and the points system. How can a person with chronic conditions still be given the same targets as a person that does not have a condition ? Everyone is placed in a box and expected to have readings according to what Discovery requires . Does not make sense to penalize someone with a chronic condition - members with chronic conditions are not given any points for trying to control conditions.
