Discovery app keeps delinking from my health app.

I’ve been a vitality member for more than 5 years but recently, especially this year I’ve been struggling to link my health app to the discovery app. I have to consistently delete and reinstall to get my health points because it keeps delinking.

This process has become very tedious and frustrating. Why is this happening?
