False Claims History

On my claims history there is an accidental damage for R5455. My last claim was for attempted hijack on 2 Oct 2021. Fair enough you are not willing to reinstate my policy after such long time and my challenges started beginning of Covid however I never had a claim in November. Can I have an amended document please
