Your web site online query function not working

Member ***. You have a contact channel in your web site at https://www.discovery.co.za/portal/medical-aid/contact-us enabling online query. Same is not working. I keep getting the following response after submitting my query: Something went wrong while sending your query. Please try again later. I have received a Final Notice n SMS from ***9, purportedly from yourselves, informing me that my "account" with you is not FICA compliant and am threatened with account suspension if not addressed. I am required to click on a given link to resolve. In the online query screen I have attached as jpeg image of the SMS which is within the size limit. I think that the SMS is a spam/****/phishing attempt but am trying online query to get your confirmation as to its validity or otherwise
