wrong registration

I am so disappointed about I just realised on weekend that registration of my son date of Birth was printed wrong on the card.; imagine my son got sick had to be admitted they were not going to cover. Because I sent a birth certificate of Amogelang. Discovery used to be the best lately disasters only. First was my meds; wrong registration so many mistakes . no this is totally not acceptable. They really risking with peoples lives. Now I am told resend birth certificate.
