Wrong bracket and over charged

I left company that was working for and they cancel my medical aid on their side then I re instate it for personal us. When I took this medical aid they did ask for proof of income.

Now that I change to personal US I contacted them and I was advice to send 3months bank statements, offer letter and 3months payslip I explain to the consultant that I have 2months payslip since I'm new to the company. I sent document many times but till today this has not been fixed instead they overcharge me. They delay the process to charge you the payslip is written the start date bank statement approve the salary what more do they want and now they don't respond my email.
