Why are you still debiting my account for the amount of R1399 when my company pays my vitality and premiums!!!!

Why are you still debiting my account for the amount of R1399 when my company pays my vitality and premiums!!!!, every month I need to get this reversed with my bank , why do you constantly debit my account when my premiums are paid by my company? can someone resolve this since it has been ongoing for months??????? you are causing me an inconvenience because I have to phone my bank to reverse this monthly.
