Vitality Active Rewards Glitches not taken responsibility for

Ever since the Vitality Open ended, there has been a noticeable increase in "glitches" in the Vitality Active Rewards program. I have now started being very vigilant with all rewards/plays etc, and have reported the following 3 issues (first reported on 5 December 2018), yet with no attempts at resolution from Vitality.

1. I acquired 2 x R100 Nando's vouchers from my Rewards on 4/12/18. Only one worked at the till, the second kept being rejected, saying it is invalid. I reported this the following day, yet received absolutely no feedback until I persevered over these past 2 weeks (with the wii codes, and even a copy of the till slip, clearly showing that only 1 voucher was redeemed). Vitality insists that both vouchers were redeemed, which is the only line the agent kept on repeating to me on the phone, despite my insistence that only one worked. All I want is my 100 points refunded. Alternatively, Vitality is welcome to show me the till slip where the 2nd voucher was redeemed (instead of just stating the time when it was "used" ie. rejected at the till)

2. 1 of my 2 diamonds randomly disappeared in December. I check the app daily, and it has always said "0 expire this week". I am not the only person to be complaining about this (on Hello Peter and elsewehere). Yet Vitality insists that they cannot do anything about it, yet they also cannot produce evidence that my app warned me if any diamonds were going to disappear. Now my second diamond is about to expire (this time the app IS warning me). The call centre has assured me that IF the first diamond is refunded, the second one can be manually re-allocated. This is in contradiction to the Vitality agent, who again kept on repeating that there is nothing she can do, as the system tells her it has expired.

3. I achieved my Driving goal TWICE in the week of 14/12/18, yet no play was issued the following week. Eventually this past week Vitality Drive says they will manually re-issue the missing play this week, acknowledging a system glitch. Again, the Vitality Drive agent informed me that this is a "common" occurence since the Vitality Open ended (one of many system glitches, in her own words)

If your very own agent acknowledges all these glitches, why can they not be rectified, without making your client feel like he is a dishonest chancer, trying to get a "free" Nandos voucher, a "free" Diamond and a "free" play? The attitude of the Vitality call centre agent was highly suggestive of this. I highly commend the Vitality DRIVE agent for her honesty. I will only make her name available if it is guaranteed it will not negatively impact her.

Hi again Vitality Thank you with your assistance with re-issuing the missing plays and missing Nandos voucher. However I have not received any feedback about the disappeared diamond in December, without any warning on my app. As expected, my second diamond has now also expired (as it is not possible to redeem just one diamond). The agent in January suggested that the second diamond will be re-instated if it is indeed found that the first one disappeared due to a system glitch. Please provide feedback.

Another 6 weeks have passed, and I am still waiting for someone to get back to me about the re-issuing of the second diamond (see details above). The re-issued first diamond is now in danger of expiring, while waiting for this to be sorted out! This is a never-ending cycle, caused by a Vitality system-glitch (by their own admission), which should have been honourably sorted out by Vitality back in December!
