Urgent Appeal – Denied Authorization for Surgical Procedure

Good day,

I am writing to formally request an exception regarding the conditional waiting period to my plan which is set to conclude on 30 November 2025. I fully understand that these waiting periods are standard practice, however I am reaching out to appeal this specific case on medical grounds, given the urgent and progressive nature of my condition.

My doctor has provided a detailed motivational letter and all relevant medical reports which clearly outlines why this surgical procedure (DECOMPRESSION, DISCECTOMY) is necessary and cannot be postponed any longer, however the request was denied.

While I acknowledge the waiting period, my condition has reached a point where all conservative treatments have failed, and the severity of my back issue has left me unable to walk, sit, or carry out normal daily activities without experiencing unbearable pain and experiencing weakness of my left foot.

This surgery is not elective, it is essential for me to regain mobility and quality of life. Without it, my condition will only continue to deteriorate.

I respectfully but urgently request that you reconsider this decision in light of the medical evidence provided and approve the surgery as a matter of necessity.
