Still waiting on Medical Aid

Last year November (during the window period) I joined LA Health (Discovery) I was told by the broker that all my paper work was in order and that i will be an active member as form 01/01/2016...but that did not happen I contacted Discovery to ask them for my membership no. at least so I could use my medical aid in case of a emergency. They than only told me that I had to complete a medical report, I went to my GP and he completed it, now they want me to fill in another report asking for my wife's weight and height and something to do with my foot... Bare in mind that I was never informed of the medical report during the month of December 2015, a whole month past and no one contacted me, and also Discovery took it upon them-self to terminate my membership with my previous medical aid and on top of that they already took they money for January from my salary, but yet (God forbid) if me or any of my family members get ill or injured I would have to go and sit at the government hospital or clinic, the month of February is almost at its end, but they still haven't contacted me to tell me what is going on...so please I need to know what will happen next.
