Refusal of claim payout.

They refuse to pay for my 4DScan which is part of my maternity benefit.I keep being told that on the invoice they can't see the referring doctor and practice number and all that information is on the invoice. I paid cash, and now I am claiming based on the covered rate, however they are not paying back amount due to me.
